It's not. It's been lengthened. If you look at the underside of the tip, there's a dent, then two more further down the lower shaft. They would match up with his fingers, should have it not been photo shopped. Other than the dents though, it's a fairly decent PS job.

The dent below the tip is because he's not cut and that's the bottom of the head. I've photoshopped a lot of pictures in my day (although it's mostly putting my face into priceless works of art rather than cocks) and this contains none of the tell-tale signs. This beast is real. Not that that is any comfort whatsoever.
